Annual Notice to Physicians and Providers

CHRISTUS Health Plan will accept written applications for participation in their Medicare Advantage and Health Insurance Exchange networks during the period of January 2, 2026, - January 23, 2026. Physicians and providers, including advanced practice nurses and physician assistants, who wish to contract with us may apply during this period.

Applications and questions can be emailed to CHP.NetworkDevelopment@christushealth.org or mailed to CHRISTUS Health Plan at 5101 N. O'Connor Blvd., Irving, TX 75039.
